We are also excited to provide affordable, convenient DOT Physicals for CDL drivers.

Our practice accepts cash, check, and major credit cards. We will be more than happy to submit your insurance forms for you, and we accept most major insurance providers. Please call our office to verify whether your specific insurance plan is accepted. Payment plans and schedules can be discussed upon request.
Cancellation
We understand that life happens and schedules can change. If you are unable to keep an appointment, we ask that you kindly call us to reschedule your appointment as soon as possible. We ask for this notice so that we can ensure you another appointment be able to offer care for others who may need an appointment.
